Adler’s Pharmacy can provide the physician’s office with pre-printed Prescription Request Forms, for various compounds listed below, which can easily be filled out and faxed to directly to us. Iontophoresis and Phonophoresis prescriptions can be delivered directly to the facility prior to the patient’s scheduled appointment.
